Poor care coordination for hospitalized patients can both increase costs and worsen outcomes. During this webinar, Prof. David Meltzer described the rationale for and design of the Comprehensive Care Physician model wherein physicians provide both inpatient and outpatient care to patients at high risk of hospitalization. He also reported on the progress on study of the model, supported by the Center for Medicare and Medicaid Innovation.
